Definition:This data element is used to clarify the process that was used to identify the patient for data collection. During the pilot project, patients may be identified as part of a random sample of all patients 18 years of age or older, OR as part of a sample of patients that were identified through a data collection log (who have already been determined to have a positive screen for alcohol or tobacco).
Suggested Data Collection Question:Was this patient record identified through the use of a data collection log (patient already screened positive for alcohol or tobacco use)?

Y Yes - This patient was identified from a data collection log that identified the patient as having screened positive for tobacco or alcohol use.

N No - This patient was identified as part of a random sample of all patients, 18 years of age or older, who had been admitted to the hospital as an inpatient.
