Definition:Documentation that patient was received as a transfer from another hospital emergency department.
Suggested Data Collection Question:Was the patient received as a transfer from an emergency department of another hospital?

Allowable Values:

Y    (Yes)   Patient received as a transfer from another hospital emergency department.

N   (No)   Patient not received as a transfer from another hospital emergency department or unable to determine from medical record documentation.

Notes for Abstraction:
  • The emergency department of another hospital includes both emergency room AND observation bed/unit stays at that hospital.
  • If a patient is transferred in from the emergency department or observation unit of ANY outside hospital, select “Yes”, regardless of whether the two hospitals are close in proximity, part of the same hospital system, have a shared medical record or provider number, etc.
  • If a patient is transferred in from a Disaster Medical Assistance Team (DMAT), which provides emergency medical assistance following a catastrophic disaster or other major emergency, select “Yes.”
