6    The patient is attending outpatient counseling

7    The patient is taking tobacco cessation medications as prescribed

8    The patient is not attending the referred outpatient counseling

9    The patient is not taking tobacco cessation medications as prescribed

10    UTD, unable to determine from medical record documentation
