| Definition: | A telephone call placed by a health care provider or other employee of the discharging institution and received by the discharged patient within 30 days post discharge for the purpose of gaining information about the patients post discharge status for tobacco and alcohol use. To meet the intent of the measures (TAM-4 and TAM-8), the telephone call must result in live contact with the discharged patient or information may be obtained when the patient returns for a clinic visit within 30 days. |

4 Attempt was made to contact patient but was unsuccessful because patient had expired, patient was incarcerated, contact number was longer valid, or the patient had no phone.
