Data Element Name: Follow-up Contact
Collected For: TAM-04, TAM-08,
Definition:A telephone call placed by a health care provider or other employee of the discharging institution and received by the discharged patient within 30 days post discharge for the purpose of gaining information about the patients post discharge status for tobacco and alcohol use. To meet the intent of the measures (TAM-4 and TAM-8), the telephone call must result in live contact with the discharged patient or information may be obtained when the patient returns for a clinic visit within 30 days.
Suggested Data Collection Question:Was a completed follow-up contact with the discharged patient made by a healthcare provider or other employee of the discharging institution within 30 days post discharge?

Allowable Values:

1    A follow up contact was made within 30 days post discharge and live contact was made with the patient

2    A follow up contact was made within 30 days post discharge and a message left on voice mail, no contact was made with the patient

3    A follow up contact was not made within 30 days post discharge

4    Attempt was made to contact patient but was unsuccessful because patient had expired, patient was incarcerated, contact number was longer valid, or the patient had no phone.

5    UTD Unable to determine from medical record documentation

Notes for Abstraction:
  • If a follow up contact was made but outside of the 30 day time frame, select value 3
  • If a follow-up contact was made and contact was made with a family member only, select value 2
  • If information was obtained in person at the time of a clinic visit within 30 days post discharge, select value 1
