Data Element Name: Referral for Addictions Treatment

Definition:Documentation that a referral was made at discharge for addictions treatment by a physician or non-physician (such as nurse, psychologist, or counselor). A referral may can be defined as an appointment made by the provider either through telephone contact, fax or e-mail, or contact information given to the patient including person to contact, number to call or fax, or e-mail address to make an appointment. The referral may be to an addictions treatment program, to a mental health program or mental health specialist for follow-up for substance use or addiction treatment, or to a medical or health professional for follow-up for substance use or addiction.
