1 Bleeding
2 No bleeding, but scheduled for invasive procedure with documentation of thrombocytopenia or platelet dysfunction
3 No bleeding, but with documentation of thrombocytopenia
4 Other
5 No documentation of clinical indication for the platelet transfusion

| Notes for Abstraction: |
- The clinical indication for the transfusion must be documented within 24 hours after the start of the transfusion.
- Select "1" if there is documentation of ‘hemorrhage’ or a 'massive transfusion protocol' was used.
- Select the first three platelet transfusion events after hospital arrival for abstraction.
- If there is an indication, but there is no documentation of "thrombocytopenia", "low platelet count" or "platelet dysfunction" by the physician, PA, or APN, select "4".
