| Notes for Abstraction: |
- Abstract Preoperative Anemia Screening associated with the elective surgical procedure selected for abstraction.
- To select “1”, confirm that a hgb or hct result was completed 14 – 45 days prior to surgery or there is written documentation of preoperative anemia screening at least a minimum of 14 days prior to surgery.
- If there is documentation that Preoperative Anemia Screening was done, but unable to determine when, select “2
- If there is documentation that the patient received iron alone or in conjunction with epogen prior to surgery, select the applicable allowable value “1” or “2”. For example, if there is documentation that iron was a preoperative medication but the patient does not know how long they have been on iron, select "2.".
