  • Documentation about the patient’s family and living situation may reflect the patient’s level of function prior to the hospital admission.
  • If documentation reflects that the patient was able to ambulate independently (no help form another person) with or without an assistive device, select value ‘0’, ‘1’, ‘2’, or ‘3’ based on the patient's level of disability.
  • If the patient was able to walk with the assistance of another person, select value ‘4’.
  • If the patient was unable to ambulate prior to the hospitalization, select value ‘5’.
  • Select the value (values 0-5) that best represents the patient’s pre-stroke level of function.
  • No value should be selected more than once.
  • If more than one value is estimated, select the highest value.
  • If a score range is estimated, e.g. 2-3, select the higher value.
  • If a pre-stroke mRS cannot be determined (UTD) from current medical record documentation, select “6”.
  • If value 6 (UTD) is selected, no other values should be selected.
