Notes for Abstraction:
  • If multiple contacts are made with the discharged patient post discharge, select the date of the contact where progress with substance use was addressed, even if this contact occured outside of the 30 day window.
  • If multiple contacts address substance use post discharge, select the date of the earliest contact.
  • If contact is made thru e-mail or letter, select the date of receipt of the patient's alochol, tobacco, or drug use post discharge status, not the date the e-mail or letter was sent.
