- History of any remote episode of documented atrial fibrillation or flutter except within 8 weeks following CABG
- PAF
- Paroxysmal atrial fibrillation
- Partial hip arthroplasty
- Partial hip replacement
- Persistent atrial fibrillation
- THR
- TKR
- Total hip arthroplasty
- Total hip replacement
- Total knee arthroplasty
- Total knee replacement
- Treatment of venous thromboembolism

- Hip fracture
- History of atrial fibrillation or flutter that terminated within 8 weeks following CABG
