Heparin 5000U SQ Q8 hrs is ordered/administered on hospital day 2. Is this acceptable to answer yes to antithrombotic administered by end day 2? The following statement is mentioned in the rationale for the measure: "Anticoagulants at doses are insufficient as antithrombotic therapy to prevent recurrent ischemic stroke or TIA"

Answer:

Heparin 5,000 units SQ is NOT acceptable for Antithrombotic Therapy By End of Hospital Day 2 (see Appendix C, Table 8.2 Antithrombotic Medications-Stroke; only IV heparin is specified). The stroke patient should receive both VTE Prophylaxis (e.g., 5,000 units SQ heparin) AND antithrombotic therapy (e.g., ASA) by end of day 2, unless contraindicated and the reason documented.
