Question: Is Xarelto an antithromotic for use in Stroke? Can it be used for antithrombotic med at discharge?

Is Xarelto approved for use with stroke as an antithrombotic? Pt had elective cardioversion 2 days prior to arrival and was sent home on Xarelto. He can to ED with chest pain, SOB & dizziness. Pt diagnosed with CVA later on the day of arrival. DO I say yes to antithromotic therapy by Day 2 if pt was taking Xarelto prior to arrival and during hospitalization? If Xarelto is ordered at discharge, do I say yes to discharge antithrombotic? Or is Xarelto an anticoagulant and therefore I would say yes to anticoagulant prescribed at discharge? IF Xarelto is ordered at discharge, is it an implied reason for no anticoagulant or antithrombotic at discharge? Thank you.

Answer:

Select 'NO' for Xarelto prescribed/administered as an antithrombotic or anticoagulant for discharges prior to January 1, 2013.

Xarelto has been added to Appendix C, Table 8.2 Antithrombotic Medications-Stroke and Table 8.3 Anticoagulant Medications-Stroke with Version 4.2, effective January 1, 2013. Beginning January 1, 2013, you may select 'YES' and use for STK-2, STK-3 and STK-5.
