Question: Must the patient refuse both pharmacological and mechanical VTE prophylaxis to count as a reason for no prophylaxis?
Patient refused Lovenox , NIHSS=0 on admission, IPC's not administered. Is this documentation sufficient to select "Yes" for reason for no prophylaxis? Is specific documentation for no IPC's required in addition to the above documentation?
Answer:
Select 'A' None for "VTE Prophylaxis" and 'Yes' for "Reason for No VTE Prophylaxis-Hospital Admission". Patient refusal of one form of prophylaxis, i.e., lovenox, is a sufficient reason.
