When VTE prophylaxis is started several days after hospital admission, should "A" be selected for VTE Prophylaxis?

Answer:

Select the value for form of VTE prophylaxis that was administered, then enter the VTE Prophylaxis Date. If the initial date of VTE prophylaxis is greater than or equal to 2 (day of or day after admission), the case will be in the denominator but not in the numerator.

Select 'A'(none/not documented/unable to determine) when no prophylaxis was administered during the hospital stay OR a Reason for No VTE Prophylaxis was documented the day of or day after hospital admission (reasons must address BOTH pharmacological and mechanical prophylaxis). Ischemic and hemorrhagic stroke patients who received VTE prophylaxis or have documentation why no VTE prophylaxis was given on the day of or day after hospital admission are included in the numerator.
