Can a chart assessment with recommendations, without actually seeing the patient, suffice for a rehabilitation assessment? What about the scenario when PT documents that the patient was off the floor but the chart was reviewed electronically. Can 'Yes' be selected for Assessed for Rehabilitation?

Answer:

This measure was developed with the intent that every stroke patient's needs for rehabilitation will be assessed. A patient assessment is more than a document review. The specifications cannot address every possible scenario; however, the data element definition (i.e., exclude (do not count as rehab assessment)a request/order for inpatient rehabilitation consult that was not performed) is clear that the assessment needs to be completed to select 'Yes'.
