Question: Stroke VTE Prophylaxis

This question has follow-up question(s): Can an addendum be added to the medical record?

If a patient receives SCD's and TED's on Day 1, then SCD's, TED's and SQ Heparin on Day 2, will this case pass or fail the STK- 1 measure? Which values should be selected for VTE prophylaxis? What day should be used for VTE Prophylaxis Date -Day 1 or 2?

Answer:

This case will pass. Select values '1' low dose unfractionated heparin (LDUH) and '3' intermittent pneumatic compression device(IPC). The VTE Prophylaxis Date is the date of initial prophylaxis or Day 1 for the case exampled above. There is one exception to this rule. If graduated compression stockings (GCS) alone are administered on Day 1, then the VTE Prophylaxis Date would be Day 2 or the day that the other form of prophylaxis was administered. When GCS are the only form of prophylaxis administered on Day 1 or Day 2, the case will fail unless there is a documented Reason for No VTE Prophylaxis present in the medical record as to why GCS were the only prophylaxis administered. Day 1 = the day of hospital admission; Day 2 = day after hospital admission.
