Question: Transmission of Continuing Care Plan (CCP)

There is documentation in the medical record that a fax was sent to the next level of care provider but does not detail (describe) what information was sent to next level of care provider. For all four data elements that constitute the CCP, would this fail HBIPS-7 since the information, "stating faxed", is not detailed enough to determine what was sent?

Answer:

For HBIPS-6, your hospital needs to define which document(s) make up the continuing care plan (CCP). As long as these document(s) contain the 4 discharge pieces (reason for hospitalization, discharge diagnosis, medications and recommendations for next level of care), the record would pass HBIPS-6. Once again, there must be documentation of which document(s) were faxed in order to pass HBIPS-7.
