Question: Patient Receiving Continuing Care Plan Only

I have a question that asks if there is documentation that the patient was referred to the next level of care provider upon discharge from a hospital based inpatient psychiatric setting. We have a discharge summary with one copy given to patient and one copy remaining on the chart. On the summary, it has a place that says Discharged to Whom (name) ______________ (left blank for nursing to fill in). Nursing writes in either self, family or facility. Does this suffice (if they wrote the place where the patient is being discharged to even though the only copy of the information was provided to the patient?

Answer:

The continuing care plan (CCP) document(s) must be transmitted to the next level of care provider in order to select "yes". Giving a copy of the CCP to the patient or family at the time of discharge does not constitute 'CCP transmitted to the next level of care provider'. There must be documentation that the next level of care provider also received a copy of the CCP in order to select "yes".
