Question: Next Level of Care Provider Same as Hospital Provider

If the next level of care provider is the same as the discharging provider, can credit be taken for the continuing care plan regardless if all the components were documented or sent?

Answer:

The Continuing Care Plan documents should still be present in the medical record within 5 days of discharge along with documentation that the next level of care provider has access to the documents. The documentation is necessary, so that a outside reviewer can make the same determination that the entire continuing care plan was present in the medical record and the provider has access to the documents.
