Question: How to Answer Questions on Advanced Stroke Tab Intent to Perform MER

We had a case where the intent was to perform a MER, but after the microwire was used, the clot moved distally and the decision was made not to engage the clot. There was no code for thrombectomy and only images were obtained during the procedure. There was a puncture time and the physician documented a TICI of 0 because no reperfusion was obtained. Why would we still fail CSTK-8,11,and 12 with no procedure code for MER and answering "yes" to attempted and "no" to failure? How should we answer those 2 questions on the Advance Stroke Tab? "Yes" to attempted and "Yes" to aborted before removal of LVO? "Yes" to attempted and "No" to aborted? "No" to attempted and "No" to aborted?

Answer:

Lesli, If there was groin puncture, select "Yes" for attempted. For the Joint Commission data element, "Failed Attempt at Thrombectomy" select "Yes". A mechanical thrombectomy procedure was intended but not completed / unsuccessful.
