Question: MER Attempted During this Encounter - Clot Dissolved

Patient is taken to angio for MER and a puncture is made. They find the clot was already dissolved, so procedure aborted and no thrombectomy performed. Do we answer yes or no to MER attempted?

Specs indicate: Yes: Taken to procedure suite and at minimum puncture was performed. No: taken to suite but did not proceed (ie clot dissolved, thus aborted.

The above example has both elements to yes and no.

Answer:

The procedure was aborted so there is probably no ICD-10-PCS Codes on Table 8.1b which is the reason for this data element; however, the procedure didn't fail. If the clot dissolved, then reperfusion to some degree was obtained. CSTK-11 captures the percentage of ischemic stroke patients who achieve TICI 2B or higher for the primary vessel occlusion within 120 minutes (>/= 0 min. and </= 150 min. of hospital arrival.

If there is a TICI score documented for this case, then the case can be considered successful and we suggest that you select "NO" for Failed Attempt at Thrombectomy. N (No There is no documentation that a mechanical thrombectomy procedure was attempted but unsuccessful or aborted before removal of the LVO, OR unable to determine from medical record documentation.

"Yes" should be selected when the thrombectomy procedure was attempted but unsuccessful, e.g, occlusion was not removed and therefore no or little reperfusion. Y (YES) There is documentation that a mechanical thrombectomy procedure was attempted but unsuccessful or aborted before removal of the LVO.

If "NO" is selected for "Failed Attempt at Thrombectomy" and there is no ICD-10-PCS code on Table 8.1b documented in the medical record, then the case will be excluded from the measure population.
