Question: Documention of Stroke education at discharge,

On the discharge summary, the physician assistant notes that the patient was educated on activation of EMS, follow up information, discharge meds, risk factors for a stroke, and the warning signs and symptoms of a stroke; however, it is not documented that WRITTEN instructions were provided to the patient. Is this acceptable documentation to support the selection of "Yes" for each stroke education component?

Answer:

No. Documentation must reflect that a copy of written materials/instructions for each of the 5 educational components was given to the patient and/or caregiver to take home. Only those patients discharged to home, home care, or law enforcement are included in the Stroke Education measure.
