Question: activity charting
How specific should the activity recommendations be? For example, should the recommendation be fairly specific stating "walk for 30 mins a day" or can it be more general stating "continue daily walks"?
Answer:
As long as the activity is individualized for the patient and is documented in the record, that will meet the intent of the measure.
