Question: Hunt and Hess & ICH Score Documentation

Our institution provides telemedicine/telestroke consultations. If our provider documents an H&H or ICH score on a patient who is then transferred to our facility, can the score from the telemedicine note count as being the first score documented at our facility?

Answer:

Scores obtained via telemedicine and documented in the medical record count for the "Initial NIHSS Score Performed". It is uncertain as to how frequently Hunt and Hess or ICH Scores are done via telemedicine. This issue was not raised during our pilot test of the measures; therefore, the telemedicine/ teleneurology note for abstraction only appears in the data element definition for "Initial NIHSS Score Performed" currently. We will consider modification of the ICH Score and Hunt and Hess Scale data element defintions to include a note about scores obtained via telemedicine for the next revision of measure specifications.
