Question: doucumentation of ich and hunt hess

In reviewing the new cstk measures we have a question on 3, the Hunt Hess or ICH score If after the time period of 6 hours (let's say patient is in the OR and is now going to the ICU…the intensivist doctor has gotten a report from the neurosurgeon, looked at documentation and the arrival imaging then documents a score, can that be taken? It would be after the 6 hours but the assessment and imaging is from arrival. Can they document …."on arrival Hunt Hess is..." ??? Thank You Teresa Daggett )

Answer:

CSTK-03 is time-sensitive measure, as are most measures in the CSTK set. Patients with a principal diagnosis of SAH and ICH who undergo an aneurysm repair procedure or surgical intervention need to have a severity assessment prior to the intervention. The time of the assessment (Hunt and Hess or ICH Score must be prior to the procedure in order to include the case in the numerator population.

For non-surgical SAH and ICH patients, the Hunt and Hess or ICH Score must be done within 6 hours of arrival time to include the case in the numerator. Scores documened outside of these parameters will not pass the meausre.
