Question: Can documentation on a pre-printed stroke order form for VTE Prophylaixs be used as a sufficient reason for no prophylaxis?

The physician documents "ambulation only" on a pre-printed order form for VTE Prophylaxis. Is this a sufficient link between ambulation/patient ambulatory and no need for VTE prophylaxis?


Since the form specifies orders for VTE Prophylaxis and the physician writes "ambulation only", it is cliearly implied that ambulation is sufficient and that no VTE prophylaxis needed, similar to the physician writing the same in a progress notes. If the form was signed by the physician, select 'Yes' for "Reason for No VTE Prophylaxis-Hospital Admission".
