Question: Why are patients that receive t-PA in the ED and then transferred to another hospital excluded from STK-4?

We use the TJC version Stroke PMT with Outcomes Database. It's excluding a patient that came into the ED, received tPA, was never admitted and then transferred to another acute care facility from the STK-4 measure. Why is this patient excluded from the STK-4 measure?

Answer:

The stroke measure set is for inpatients (Refer to the Stroke_List pdf in specifications manual). http://www.jointcommission.org/PerformanceMeasurement/PerformanceMeasurement/Current+NHQM+Manual.htm
