This is follow up to a prior question: "
Data Abstraction for
A patient presented to the ED with complaints of HA. At 0235 notes indicate that "patient states HA almost gone". While being interviewed by the physician, "she began having right- side weakness, no longer answering questions and mouth drooping." Do I take the time she discovered symptoms before going to the hospital ED or the time of symptom onset in the ED?
Answer:
"Last Known Well" has two allowable values, 'Yes' and 'No'. Both a date and time of last known well are needed to select 'Yes". To determine whether to select "Yes" or "No" for Last Known well, the clinician must determine when the patient was last known to be at their baseline or usual state of health and not symptom onset. Last Known Well occurs prior to hospital arrival.
