Question: Discharge Medications and Indication for Use

Would you clarify for HBIPS-6 and HBIPS-7 the documentation required to satisfy the discharge medication ‘indication for use.’

For example, if an antidepressant is prescribed, ‘Zoloft,’ must they clarify ‘for depression’ to meet the requirementfor the measure? For ASA or statin, etc is 'indication for use' also required?


All medications must have the names, dosage and indication for use listed in the continuing care plan. The indication for use can be as short as one to two words, but must be present for all medications. According to Medication Management Standard, MM.04.01.01, all prescribed medications must have a diagnosis, condition or indication for use listed in the medical record; therefore, this information should be readily available in the medical record at the time of discharge when completing the continuing care plan.
