Question: mRS

For the 90 day follow up mRS, I am finding documentation such as pt. back to baseline, symptoms completely resolved, or a NIHSS 0. So far there is no actual mRS number documented. Do I continue to use mRS 8 or can an mRS number be assigned by the abstractor as interpreted from the documentation exampled above? This is the last step to completion of the comprehensive stroke project. Thank you


It was not intended for abstractors to infer a mRS. mRS interviews should be done by a trained evaluator as noted in the rationale for CSTK-02. A trained evaluator should be documenting an actual score 0-6 (i.e., 0-6 is the mRS scale and we/TJC added allowable values '7' and '8' to account for situations when the patient is lost to follow-up or no one did/tried to do a mRS at 90days.

The CSTK-02 measure as currently wriitten is a process measure. The focus is that a mRS score was done/obtained at 90 days and not on the actual score obtained for the patient. For the pilot, if a score was obtained, go ahead and infer the value from the documentation that you have available and enter the mRS score date. It is the "mRS Date" data element that will determined if a case is included in the numerator or not. If '8' if selected for Modified Rankin Score data element, the case will be excluded from the measure.)
