Question: Activity specifics

Activity recommendations are charted and verbally told to the patient. If the activity plan was included with their outpatient visit instructions, would that be sufficient (i.e. not a separate document for individualized activity needs)? And if so, would it have to be included on every heart failure clinic outpatient visit?

Answer:

While the data element does indicate "Activity Recommendations Document" there actually does not have to be a specific "document" just written instructions that individualized activity recmmendations tailored to the patients needs is given to the patient and documented in the outpatient record.
